As an MDT Co-ordinator, I ensure that all relevant information is shared between Multi-Disciplinary Team members so they can make informed decisions about the best course of treatment for each patient. I also track patients through their care pathway, book appointments, send referrals to other agencies, identify potential breaches in waiting times and ensure that all relevant patients are discussed at MDT meetings. I also manage our Service Facebook page, and record and edit exercise videos with our Exercise Physiologist which aims to help patients in their Long Covid recovery. Since starting with the team, I have implemented numerous changes aimed at improving communication and engagement with patients and other agencies, I capture data to identify health inequalities
